Key Responsibilities
  • Undertake assessments of risk and care needs for service users and carers, ensuring that appropriate action is taken, monitored and reviewed in line with legislation.
  • Manage a caseload of varying complexity, receiving and responding to referrals from multiple sources.
  • Conduct Community Care and Integrated Care Programme Approach assessments and risk assessments to protect the safety and welfare of individuals.
  • Complete statutory reviews, ensuring compliance with relevant legislation, policies and procedures.
  • Work collaboratively with service users, carers and other professionals to assess needs and agree support plans.
  • Engage with individuals who may refuse services but are at risk to themselves or others, using sensitive negotiation skills.
  • Manage crisis situations effectively, applying appropriate legislation and available resources.
  • Fulfil statutory responsibilities for the assessment of carers, including young carers, and develop carers’ care plans.
  • Prepare and present reports, assessments and statements to support decision-making across teams and agencies.
  • Maintain accurate, up-to-date client records in accordance with legislation, policy and procedures.
  • Facilitate needs-led service provision, working with providers in the statutory, voluntary and independent sectors.
  • Plan and agree service responses and resources, developing realistic care plans to meet individual needs.
  • Negotiate the purchase of care provision where appropriate.
  • Develop and maintain effective joint working relationships with primary care, housing, police and other agencies.
  • Establish and maintain strong professional relationships with service users, their families and other professionals to enable effective partnership working.
Essential Requirements
  • Degree or Master’s in Social Work and SWE registration.
  • 2+ years of post-qualification experience in Adult Services.
  • Strong knowledge of UK social care legislation, statutory responsibilities and safeguarding procedures.
  • Ability to manage complex caseloads and work in high-pressure environments.
  • Right to work in the UK.
