Adults Care Home Support Worker within Learning Disabilities Team/Ageing Health Needs and Mental Health in Oxfordshire UK earning £13.47 per hour Hourly.

What you will do:

• Ensure that all individuals are fully involved with their person‑centred planning, with their plans developed and shared with them and their support network.
• Contribute to team duties including personal and healthcare provision, ensuring statutory requirements are fulfilled.
• Support people with Autism by developing an environment and approach that meets their unique needs and empowers them with choice and control over their lives.
• Assist individuals or groups in planning and preparing for various activities, including sport, leisure, recreation, and college courses.
• Support Intensive Interaction and other Total Communication methods identified by local services and the individual’s network.
• Be responsible for health and safety, vehicle safety checks, and ensure accountability for administering medication and finances.
• Undertake transport duties and driving responsibilities for services and community activities.
• Work within safeguarding policies and procedures, ensuring timely escalation of any necessary ‘Safeguarding Alert’.
• Promote community integration, utilising community resources to help individuals meet their identified outcomes.
• Engage in cross‑agency collaboration to provide person‑centred support.
• Take ownership of professional development and share learning across the organisation.
• Participate in service development and performance enhancement activities.
• Contribute to team success against performance targets, ensuring robust implementation of feedback through the Council’s Comments and Complaints policy.
