An exceptional opportunity has opened for a Social Worker to join The Home First Neighbourhood South Team in Oxford, with a focus on mental health hospital discharges. In this full-time, ongoing locum role, you’ll champion a social and rights-based approach within a dynamic multidisciplinary team—ensuring individuals receive timely and appropriate support to thrive upon discharge.

What you will do:

Provide advocacy and holistic social care to people discharged from mental health hospitals.

Promote person-centred recovery, autonomy, and self-directed change.

Conduct Care Act Assessments, Mental Capacity Assessments, S117 Assessments, and contribute to Best Interest Decisions.

Draft and present Social Circumstances Reports to Mental Health Tribunals.

Lead safeguarding and risk assessment processes with confidence and clarity.

Apply frameworks such as the Mental Health Act and Mental Capacity Act to support legally sound care planning.

Liaise with carers, families, and other professionals to deliver well-rounded support.

Participate in multidisciplinary reviews and strategy meetings at the Fullbrook Centre.

Design effective support plans aligned with individual needs, goals, and preferences.
