An exceptional opportunity has opened for a Social Worker to join The Home First Neighbourhood South Team in Oxford, with a focus on mental health hospital discharges. In this full-time, ongoing locum role, you'll champion a social and rights-based approach within a dynamic multidisciplinary team—ensuring individuals receive timely and appropriate support to thrive upon discharge.

What you will do
  • Provide advocacy and holistic social care to people discharged from mental health hospitals.
  • Promote person-centred recovery, autonomy, and self-directed change.
  • Conduct Care Act Assessments, Mental Capacity Assessments, S117 Assessments, and contribute to Best Interest Decisions.
  • Draft and present Social Circumstances Reports to Mental Health Tribunals.
  • Lead safeguarding and risk assessment processes with confidence and clarity.
  • Apply frameworks such as the Mental Health Act and Mental Capacity Act to support legally sound care planning.
  • Liaise with carers, families, and other professionals to deliver well-rounded support.
  • Participate in multidisciplinary reviews and strategy meetings at the Fullbrook Centre.
  • Design effective support plans aligned with individual needs, goals, and preferences.
