Adult Social Work Team Leader, Hospital Discharge and Post-Discharge Assessment Team, West Yorkshire. Pay rate £40 per hour, 30 hours per week. Contract role, Hospital Team. Pertemps are recruiting for experienced Adult Social Worker in West Yorkshire. This is work as the Team Leader for a Hospital Discharge and Post-Discharge Assessment Team. There is an expectation to be on-site for 3 days across Calderdale Royal Hospital, Huddersfield Royal Infirmary, and Princess Buildings in Halifax.
