Job description

We are excited to introduce a new part‑time role designed to strengthen support within the PCNs adult mental health service. The purpose of this position is to provide practical, hands‑on assistance to patients as they progress through their treatment. The role will include helping individuals set achievable goals, supporting them in following through with identified activities or referrals, and offering motivational coaching to maintain positivity and engagement.

The role may involve accompanying patients to appointments or community activities and fostering connections between patients, GP practices, and local services to enhance community integration. Working closely with our Mental Health Practitioner this role will play a key part in ensuring patients feel supported and empowered throughout their recovery.

We’re recruiting for a new part‑time role within our adult mental health service, focused on providing practical support to patients throughout their recovery journey. From helping individuals set achievable goals to accompanying them to appointments and fostering community connections, you’ll play a vital role in empowering patients and strengthening local support networks.

If you have experience as a Health Coach or Social Prescribing Link Worker (Mental Health), and you’re passionate about improving mental health outcomes, we’d love to hear from you. As this is a new position, we are seeking candidates with either a Health Coaching background or experience as a Mental Health Social Prescribing Link Worker, bringing the right mix of skills and enthusiasm to make a meaningful impact.
