Job description

Are you passionate about making a positive difference to people’s lives? Sheffield City Council is looking for motivated and compassionate Social Care Practitioners to join our Adult Social Care service and help us deliver high-quality, person-centred support to adults across the city.

This is an exciting time to join us. We are strengthening and developing our services so that people receive the right support, at the right time, in a way that promotes independence, choice and wellbeing. We are looking for people who are enthusiastic about working creatively, embracing change and contributing to a modern, strengths-based adult social care model.

Our Home First – First Contact Team provides a welcoming and responsive first point of contact for adults, families, carers and professionals seeking adult social care support in Sheffield.

The team offers a wide range of support, including information and advice, signposting to community services, preventative interventions, and the completion of person-centred assessments and support plans. Our focus is on understanding what matters to each person and supporting them to achieve outcomes that are meaningful to them.

You will work in a supportive and committed team that values collaboration, professional curiosity and respectful practice. We are looking for practitioners who can build positive relationships, recognise people’s strengths and networks, and support safe, proportionate and effective decision-making.

In this role, you will need to be organised, flexible and confident in managing your own workload, while also working closely with colleagues and partner agencies to provide a joined-up service for the people of Sheffield.

We are seeking applicants with recent experience in adult social care or a related field, good IT skills, and an understanding of key legislation and practice areas including the Care Act 2014, the Mental Capacity Act 2005 and safeguarding. Strong communication skills, excellent customer care and a commitment to collaborative working are essential.

You will receive a positive induction, ongoing support and opportunities to develop your skills and confidence within the role. We also offer flexible working arrangements that support a healthy work-life balance and help our staff to thrive both inside and outside work.

Sheffield is a vibrant, welcoming and diverse city, with a rich cultural life, strong communities, excellent transport links, good schools and easy access to green spaces and the Peak District.

Sheffield City Council is a great place to work and is committed to supporting, developing and valuing its workforce. Benefits include:

  • Flexible working policies
  • Support to parents and carers
  • A Local Government Pension scheme
  • Hybrid working
  • A good level of holiday entitlement and much more

Our strong workforce development offer promotes continuous learning, reflective practice and staff wellbeing, with a clear focus on improving outcomes for adults, families and carers.

We are committed to supporting a professionally skilled, diverse and dynamic workforce. Through our Progression Framework and blended learning and development programme, staff are encouraged to grow, share knowledge and contribute to excellent, creative and inclusive practice.

If you enjoy supporting people to achieve their goals, working with others to find positive solutions, and being part of a dedicated and forward-thinking team, we would be delighted to hear from you.

Further details about the responsibilities and requirements of the role are included in the role summary and job profile.

This is a fantastic opportunity to join a service that is ambitious for the people of Sheffield and supportive of the staff who make that work possible. We welcome your application.

Under the Disability Confident Scheme, disabled applicants, who meet the essential criteria of this job, are guaranteed an interview.

We are committed to safeguarding and promoting the welfare and safety of children and vulnerable adults and expect all staff to share this commitment. All successful candidates will be required to complete an Enhanced Disclosure and Barring Service Check.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
