Contract role, Social Care Connect Team

Pertemps are recruiting for an experienced Adult’s Social Worker in the Scunthorpe area.

This is to work in a Social Care Connect Team.

Responsibilities

• Working as part of the Social Care Connect Team which will include home visits and completing Care Act Assessments, working within the community and hospital integrated discharge team based at Scunthorpe General Hospital.

Requirements

• Completion of MCA/BI as necessary.

There is plenty of hybrid working available.
