  • Screen all duty enquiries within the team, with support from the management team as required.
  • Work within community hubs to provide support and services.
  • Take a strengths‑and‑asset based approach, supporting individuals using the strengths they possess and their network of support.
  • Collaborate with Children's Services, SEND and other partners to coordinate care and facilitate smooth transitions.
  • Promote independence, choice and control for service users by tailoring care to assessed needs and strengths.

    1 x permanent contract – 28.5 hours part time (salary pro‑ra). Competitive salary – pro‑rated for part‑time role.
