Job summary

A leading mental health organization in Birmingham seeks an Admissions Avoidance Coordinator to provide individualized support for individuals at risk of crisis. This role focuses on collaboration with healthcare staff and community hubs to strengthen wellbeing for ages 18-25. Qualifications

Role Overview

We are a leading provider of mental health services in Birmingham and the West Midlands.

We now have an opportunity for an Admissions Avoidance Coordinator to join us to work closely with people who have experienced mental health difficulties and are being supported through BSMHFT Children’s and Young People Community Hubs and Crisis services.

The main aim of the service is to offer an individualised service with a focus on the prevention of hospital admission and provide support for service users.

Responsibilities

You will work closely with the other Discharge Coordinators providing support cover when required and you’ll work closely with BSMHFT CYP staff to identify individuals who are at risk of escalation because of hardships including social and environmental factors.

These individuals will be supported via the service through a person‑centered approach that focuses on maintaining and strengthening the person’s wellbeing and enabling people to build their own resilience reducing crisis escalation.

Qualifications
  • Experience of working in a paid role in the mental health field.
  • Experience of supporting people age 18‑25 to be in control of their lives.
  • Knowledge and experience of carrying out needs & risk assessments.
  • Good overall understanding of the social determinants of poor mental health and interventions to effect positive outcomes.
