Job Summary

We are looking for an Admin support/Care Navigator to join our experienced team. The role involves sign‑posting patients to the appropriate healthcare professional or service, working as part of the practice's multidisciplinary team. In addition to core support duties, you will undertake reception & admin tasks, ensuring a polite and professional service to the patient population.

Responsibilities
  • Assisting in the administration and support operations of the practice.
  • Managing requests from external organisations such as the local police, solicitors, insurance companies, DVLA and other agencies (PMA).
  • Managing the referral process and dealing with queries relating to referrals in a timely manner.
  • Managing registrations to included processing, queries and deductions.
  • Working with DPM on the practice patient recall system to update medical records and arrange appropriate review appointments to achieve maximum points in QOF, ES, LIS, LES, DES and target‑driven recall programmes.
  • Summarising incoming medical records.
  • Managing the patient information on display to include website, TV screen, social media accounts and waiting room noticeboards.
  • Acting as focal point for the practice in managing requests from external organisations, including generating invoices for patient PMA requests.
  • Acting as building fire marshal, ensuring evacuation lists are current and maintaining the visitor log.
  • Maintaining the reception area, noticeboards, and supplies.
  • Supporting clinical staff with general tasks as needed.
  • Managing the movement of patient medical records and envelope management.
  • Completing opening and closing procedures in accordance with the duty rota.
  • Supporting pharmacist(s) in the management of prescriptions.
  • Action of incoming emails and updating EMIS with read‑code data.

  • Process and signpost patients to appropriate services.

  • Initiating contact with and responding to requests from patients, team members and external agencies.
