RightcareBarnsley is a Single point of access and Clinical Advisory Service based inBarnsley, we work collaboratively with Barnsley Hospital, SWYT CommunityServices, Barnsley ICB, Local Authority and Primary Care services. We need aRegistered Nurse to join the well-established team of Nurses and call operatorsto support our work with Barnsley Virtual wards.

TheNHS definition of a VW ward is a safe and efficient alternative to NHS beddedcare that is enabled by technology. VWs support patients who would otherwise bein hospital to receive the acute care, monitoring and treatment they need intheir own home this includes preventing avoidable admissions and supportingearly discharge out of the hospital. Virtual Wards are a key element of the UECdelivery and Winter planning.

Aspart of the national Operational Planning Guidance the introduction of VirtualWards (VW) across system is seen as a priority for the NHS. Initially, thefocus is for each PLACE to develop a VW to support patients with AcuteRespiratory Infection (including Covid patients) and Frailty in their routineplace of residence. The overall national ambition is for systems to providebetween 40-50 virtual ward beds per 100k population by December 2023 forBarnsley this equates to 106-132 beds. The expectation is that VWs will beoperational seven days a week, 12 hours a day, with an out of hours provisionfor any patients on the VWs that require support.

Main duties of the job

You will assist the Operational Manager in the provisionand development of Barnsley Virtual Wards, to ensure safe, effective andindividualised support to patients discharged early onto a Virtual Ward orstepped up to support hospital avoidance from a community setting.

You will consistently maintain quality patient carefacilitating improvements and ensuring a good patient experience and managedaily operations of caseloads, successfully onboard patients onto the service,triage new referrals and support patient escalation when necessary.

As a Registered Nurse your work will be split between the RightCare call centre based at the Kendray Hospital site and Barnsley Hospital. Whilst in the call centre you will be responsible for contacting Virtual Ward patients via telephone, digital monitoring, assessing and participating in the virtual MDT meetings where patients are discussed with Community, Hospital and Social Care colleagues. You will spend part of your working week at Barnsley Hospital assessing patients who have been referred to the Virtual ward and processing the admission. Community Nurses and other MDT members will provide direct patient care in the patients own home, the RightCare Virtual ward Nurses (this post) will remotely support them to do so.
