Job summary

A community support organization in Northern Ireland is looking for an Accommodation and Community Support Worker. Qualifications

• Identify client-specific risks and create support plans.

Responsibilities

• Meet the needs of service users to promote independence.
• Provide support and advice to users.
• Assess service users' needs and risks.
• Develop and monitor outcome-based support plans.
• Liaise with external agencies for integrated services.
• Act as an advocate for clients.
