H2 Health is seeking a Physical Therapist for our outpatient Columbia, KY clinic. You will evaluate patients, develop individualized plans of care, and deliver evidence-based interventions to improve mobility and quality of life.

Formación

• DPT from CAPTE‑accredited program.
• Kentucky Physical Therapist license or eligibility.
• CPR certification where applicable.
• Strong communication and interpersonal skills.
• Commitment to high‑quality, patient‑centered care.
• New graduates encouraged to apply.

Responsabilidades

• Evaluate patients and develop individualized plans of care.
• Provide evidence-based PT interventions to improve mobility and function.
• Treat a variety of orthopedic and musculoskeletal conditions.
• Collaborate with Physical Therapist Assistants and support team.
• Educate patients and caregivers on treatment plans and home programs.
• Accurately document evaluations, progression, and outcomes.
• Build meaningful patient relationships with compassionate care.
