You will ensure care processes, take on case responsibility, and coordinate internal/external services, requiring independence, Spitex experience, and IT proficiency. A driver's license and bike mobility are essential.

Qualifications

• Degree as Dipl. Nursing Specialist HF/FH.
• Experience in Spitex preferred.
• Independent and responsible working style.
• Proficient in using IT programs (MS Office, possibly RAI-HC).
• Mobile by bike and holds a driver's license.
