Qualifications

• Two years recent relevant experience working with vulnerable populations.
• Experience working with youth populations (15-30) and housing support programs is an asset.
• A valid Ontario Drivers License with access to a reliable vehicle.

Responsibilities

• Support establishment and maintenance of stable affordable housing for clients.
• Provide professional case management services within a rehabilitation model.
• Implement treatment/care plans focusing on wellness and independence.
