Job summary
A community-based organization is seeking an Assertive Community Treatment Intensive Case Manager to support homeless youth in Peel Region. The role involves conducting assessments and facilitating care plans to help youth achieve independence. Preferred qualifications include a Bachelor's degree and experience with urban homeless populations. This position requires a valid Ontario Driver’s License and the ability to work flexible hours.
Qualifications
- 2-3 years of experience with urban homeless populations.
- Valid Ontario Driver’s License and acceptable drivers abstract.
- Must be fully vaccinated against COVID-19.
Responsibilities
- Establish and maintain relationships with assigned youth.
- Conduct clinically informed biopsychosocial assessments.
- Facilitate necessary services to address complex care needs.
