Job description
Position Summary

Performs all intake and scheduling duties and responsibilities for scheduled weekend position hours. Coordinates and prioritizes referrals and schedules with clinicians, patients/families, and referral sources to ensure optimal patient care and efficient utilization of services. Enters home health & hospice referrals and consults with administrator on call during after hours and on weekends.

Essential Job Requirements
  • Education: High School Diploma required or GED Equivalent.
  • Experience: Prior home health care, referral intake, or relative work experience in health care industry required.
  • Required skills: Must have proven ability to interact with individuals at all levels of the organization, have strong interpersonal skills, be able to handle and maintain confidentiality and have strong attention to detail. Must be able to work in a fast‑paced environment with demonstrated ability to juggle multiple competing tasks and demands.
  • Preferred skills: Home Health/Hospice experience preferred.
