A healthcare organization in Ontario, Canada is looking for a candidate to perform intake and scheduling duties for weekend positions. The ideal applicant should have a High School Diploma or GED, prior home health care experience, strong interpersonal skills, attention to detail, and the ability to thrive in a fast-paced environment. This role ensures optimal patient care and collaboration with clinicians and patients. This position may involve working with sensitive information and requires confidentiality.
Performs all intake and scheduling duties and responsibilities for scheduled weekend position hours. Coordinates and prioritizes referrals and schedules with clinicians, patients/families, and referral sources to ensure optimal patient care and efficient utilization of services. Enters home health & hospice referrals and consults with administrator on call during after hours and on weekends.
