Qualifications

  • Registered Nurse must hold a valid license in Ontario.
  • Must have Fundamentals of Palliative Care or equivalent.
  • Three years experience in community or acute care with focus on palliative care preferred.

Responsibilities

  • Manage and coordinate care; educate and advocate for clients.
  • Provide nursing care for stable and unpredictable health conditions.
  • Develop client-centered care plans based on assessments.
  • Collaborate with multidisciplinary teams and mentor other care providers.
