Job summary

A not-for-profit organization in Chatham is seeking a Transitional Support Services Worker to assist clients during their hospital discharge and home transition. Responsibilities include providing companionship, managing errands, and maintaining client confidentiality. Ideal candidates will have Standard First Aid and CPR-C certification, fluency in English, and a valid Ontario driver's license. This casual position offers work primarily in client residences during the day.

Title : Transitional Support Services Worker, Home Care Support

Employee Working Location : In person (Chatham-Kent, Ontario)

In this role, you will:
  • Provide support to client during hospital discharge process and during the transition from hospital to the client’s home in accordance with established service plan.
  • Complete basic errands and home management duties as required, in accordance with established service plan, i.e., preparing nutritious meals while safely and hygienically storing and handling food and maintaining the comfort and safety of the client’s immediate environment.
  • Complete risk analysis of home using standardized Risk Assessment tool.
  • Facilitate client and caregiver engagement, referrals to Community Support Services and discharge process as per program guidelines.
  • Identify and report any unsafe equipment and situations, handle and store hazardous products safely (WHMIS).
  • Provide companionship to clients within therapeutic boundaries.
  • Observe and report any concerns related to the client or their environment to the Program Coordinator.
  • Maintain confidentiality of clients, caregiver/family, and the Canadian Red Cross Society.
What we are looking for:
  • Minimum qualifications of Standard First Aid and CPR-C.
  • Fluency in English is required, French is an asset.
  • Previous experience with elderly clients is an asset.
  • Interpersonal skills typically gained through daily living skills: dependable, punctual, ability to manage stress and interacts well with others.
  • Ability to follow care plan as identified by the appropriate funder agency.
  • Self starter and able to work in an environment with ambiguity.
  • Proven analytical and problem-solving skills, ability to be a supportive and productive team member.
  • Valid Ontario driver's license and reliable vehicle.
Working Conditions:
  • Duties are principally conducted in the residencies of the clients.
  • Hours of operation are from 8.30 am to 4.30 pm, Monday to Friday.
  • The work can require irregular hours, but never outside of hours of operation.
  • Please note that relocation funding is not available for this position.
