A leading mental health support organization in Windsor is seeking a Supportive Housing Case Manager for youth to help establish and maintain stable housing. This role involves professional case management services, collaboration with clients, and a focus on recovery and independence. Candidates should have a Bachelor of Social Work or relevant educational background, along with experience in mental health and housing support programs.

Under the direction of the Manager Housing Facilities and Employment Services the Supportive Housing Case Manager-Youth is responsible for supporting the establishment and maintenance of safe stable and affordable permanent housing for individuals who have a serious mental illness and are homeless or at risk of becoming homeless.
This position provides professional case management services within the framework of a client directed clinical / rehabilitation case management model. The position provides a continuum of functions including: assertive outreach, building therapeutic relationships, developing care plans and outcomes with clients, symptom management, life skills teaching, supportive counseling, client and systems advocacy, family support and crisis intervention. Services are delivered to individuals and their families who may reside with them.
Through an integrated support model the incumbent will work in conjunction with their Housing Team Colleagues to support clients in maintaining appropriate and sustainable market based housing with community Landlords primarily within the Homeless Initiative (HI) Supported Housing Program. The positions focus is on overall wellness tailoring service delivery toward recovery resilience and self-sustenance in a stabilized housing environment. Notwithstanding other client directed goals a service delivery outcome of independence is a prime directive for the position.
The position is responsible for the following:
Two (2) years recent relevant experience working with vulnerable populations in the judgement of the employer. Experience working with youth populations (15-30) and housing support programs is an asset.
Comprehensive knowledge of mental illness and treatments; comprehensive knowledge of functions and principles of case management; thorough knowledge of psychotropic drugs; knowledge of relevant mental health policy legislation and regulations; knowledge of community resources; knowledge of the Residential Tenancies Act and related legislation and regulations; a sound awareness and knowledge of community resources as they relate to socialized housing; proficiency required with MS Windsor programs i.e. Word Excel Outlook and PowerPoint; proficiency in French language would be considered an asset
Hours of Work: Thirty-Five (35) hour work week. The agency's regular hours of operation are Monday to Friday 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.
Salary range: Grade 8 $35.71 - $42.25 per hour
Per 2022 - 2026 Collective Agreement
Applicable shift premiums are paid in accordance with the Collective Agreement Article 17.11.
