Team Lead Community Nurse

Main duties of the job

We are seeking experienced and dedicated nurses with relevant clinical skills who are ready to embrace the unique challenges and rewards of community nursing. Candidates wishing to move into a community setting with advanced clinical skills will be considered. Those without the specialist District Nurse qualification or an advanced clinical skills module may be considered for trainee posts and supported to achieve the SPDN qualification.

  • Use the named nurse model to spend more time with patients, promote self‑management, and improve patient lives.
  • Have a full UK driving licence, drive your own car, and claim mileage costs or access the NHS car lease provision after probation.
  • Make independent clinical decisions and adapt to the fast‑paced environment.

Person Specification
Qualifications, training and experience
  • Level 6 Degree or equivalent qualification, or equivalent knowledge through experience plus a nursing diploma.
  • Experience of managing complex caseloads and supporting/ mentoring others.
  • Registered with the Nursing and Midwifery Council.
Knowledge and skills
  • Excellent verbal and written communication skills.
  • Customer‑focused approach – providing the best possible experience for patients, clients and their families, and representing the Trust positively.
  • Proficiency in IT systems and databases.
  • Awareness of NHS developments, National Service Frameworks, and NICE guidance.
  • Attention to detail and ability to spot potential problems.
  • Up‑to‑date clinical practice, evidence‑based procedures, and clinical effectiveness frameworks.
Working environment
  • A full, valid UK driving licence and the ability to travel independently are essential.
