Position: Full-time (1.0 FTE) 37.5 hours/week; vacancy replacement
Schedule: 5 days per week with evenings and some weekends as required
Location: In the community (people’s homes) and on-site at the hospice

About Dorothy Ley Hospice

Dorothy Ley Hospice (DLH) has been caring for community members in Etobicoke, West Toronto and Mississauga for more than 35 years. As one of Ontario’s oldest and most established hospices, we’re helping to lead the future of hospice palliative care. We offer community and hospice residential palliative care, as well as grief and bereavement support. Our compassionate team of palliative care physicians, nurses, staff, and volunteers provides support and care to over 3,000 individuals annually.

The opportunity

Dorothy Ley Hospice is seeking a qualified Supportive Care Coordinator to join its interdisciplinary Community Team. Guided by the Hospice’s Shared Purpose, Vision, and Values, the Supportive Care Coordinator supports individuals living with palliative illness at home and assists their family caregivers. In this role, you will help individuals and their loved ones navigate the palliative care journey and develop personalized care plans that reflect their needs, preferences, hopes, and goals. This person‑centred approach promotes respect, dignity, and choice. You will work closely with Ontario Health at Home, the DLH Community Physician Team, and the DLH Community Team to provide holistic end‑of‑life care in the home and report directly to the Director, Community Programs and Quality.

Key responsibilities

• Support individuals through information and referral with systems navigation and advocacy.
• Assess service and support needs, then partner with individuals and families to develop holistic care plans aligned with identified goals.
• Implement care plans by connecting individuals and families to Hospice services, including spiritual care, wellness programs, in‑home volunteers, caregiver support, and residential care—and coordinating access to external health and social services as needed.
• Research and connect clients to external organisations and programmes that support their needs, including income support, advance care planning, and mental health services.
• Provide supportive counselling and psychosocial support to individuals at end of life and their families, building trust to explore sensitive issues related to serious illness.
• Provide education about end‑of‑life care tailored to each individual and family.
• Build and maintain relationships with community groups and healthcare partners and collaborate with Ontario Health at Home and the Dorothy Ley Community Physician Team to deliver integrated home‑based care.
• Deliver culturally responsive care grounded in inclusion, equity, and diversity.
• Support hospice initiatives that improve access to care and reduce barriers.
• Work with the Community Team and management to develop or adapt programming that is relevant and appropriate for diverse communities.
• Support outreach activities that educate community members and health‑care providers about hospice palliative care and Dorothy Ley Hospice services.

Qualifications

• Registered Social Worker (RSW); BSW or MSW preferred.
• At least three years of experience providing psychosocial support in palliative care.
• Experience in case management and connecting clients with community resources.
• Experience working in community settings; trauma‑informed care experience is an asset.
• Experience providing hospice palliative care.
• Comfort working with individuals living with life‑limiting illness and leading conversations about palliative illness, dying, and death.
• Knowledge of community resources relevant to hospice care.
• Commitment to health equity, inclusion, diversity, and reconciliation.
• Strong interpersonal and partnership‑building skills.
• Excellent active listening and oral and written communication skills.
• Ability to work independently and manage priorities effectively.
• Commitment to protecting the privacy and confidentiality of those served.
• Experience collaborating with health‑care partners, including volunteers.
• Strong time management and organisational skills.
• Proficiency in Microsoft Office and data tracking.
• Experience maintaining accurate client records.

Position Requirements

• A Police Vulnerable Sector Check and proof of vaccination are required as conditions of employment.
• Comfort and ability to provide in‑home visits are essential to this role.
• A valid driver’s licence and access to a reliable insured vehicle for business use are required.

Compensation Highlights: Salary range: $33.33 hourly to $41.02 hourly; 15 days’ vacation, 5 personal days, competitive health and dental benefits (after probation); participation in HOOPP (Health Care of Ontario Pension Plan)
