Position: Full-time (1.0 FTE) 37.5 hours/week; vacancy replacement
Schedule: 5 days per week with evenings and some weekends as required
Location: In the community (people’s homes) and on-site at the hospice
Dorothy Ley Hospice (DLH) has been caring for community members in Etobicoke, West Toronto and Mississauga for more than 35 years. As one of Ontario’s oldest and most established hospices, we’re helping to lead the future of hospice palliative care. We offer community and hospice residential palliative care, as well as grief and bereavement support. Our compassionate team of palliative care physicians, nurses, staff, and volunteers provides support and care to over 3,000 individuals annually.
Dorothy Ley Hospice is seeking a qualified Supportive Care Coordinator to join its interdisciplinary Community Team. Guided by the Hospice’s Shared Purpose, Vision, and Values, the Supportive Care Coordinator supports individuals living with palliative illness at home and assists their family caregivers. In this role, you will help individuals and their loved ones navigate the palliative care journey and develop personalized care plans that reflect their needs, preferences, hopes, and goals. This person‑centred approach promotes respect, dignity, and choice. You will work closely with Ontario Health at Home, the DLH Community Physician Team, and the DLH Community Team to provide holistic end‑of‑life care in the home and report directly to the Director, Community Programs and Quality.
Compensation Highlights: Salary range: $33.33 hourly to $41.02 hourly; 15 days’ vacation, 5 personal days, competitive health and dental benefits (after probation); participation in HOOPP (Health Care of Ontario Pension Plan)
