Job summary

Mountain Plains Community Services Society of the North (MPCSSN) is hiring a full-time Support Liaison for the Horizon Home in Sexsmith. You will model healthy relationships, design activities, and support youth through structured, relationship-based programming.

You will monitor goals, coordinate community supports, and collaborate with the Program Manager and stakeholders to deliver trauma-informed services.

Qualifications

  • Proof of Degree or 2 Year Diploma in human Services.
  • Valid Alberta Class 5 Driver’s License.
  • Driver’s Abstract.
  • Clear Criminal Records Check.
  • Clear Children’s Service Intervention Record Check.
  • Vehicle Insurance stating $2 million liability and access to a vehicle.

Responsibilities

  • Create a visual daily schedule based on program and individual needs.
  • Monitor and track person-centred goals with the program team.
  • Assist persons-served in building community supports and family involvement.
  • Coordinate all support strategies and monthly activity reports with the Program Manager.
  • One weekend a month on-call for the home program.
  • Collaborate with stakeholders (funding bodies, RCMP, justice system, community).
  • Champion trauma-informed, therapeutic programming across the Agency.
  • Collaborate with the Support Liaison team on Agency-wide projects and events.
  • Attend meetings, case-conferences and training as delegated by the Program Manager.
  • Participate in monthly supervision and staff meetings.
  • Liaise with families to ensure holistic service delivery.
  • Support onboarding processes with Agency tours.
  • Complete other delegated tasks by Leadership.

We are currently seeking dedicated Support Professional who will serve as positive role models to support, lead, ad encourage young people. You will have the chance to model healthy relationships, encourage positive behaviour, and design activities to promote the overall well-being. Mountain Plains focuses on creating structured, engaging, and relationship-based environment where individuals can build skills, stabilize, and move forward. The Support Liaison plays a critical role in bringing programming to life and supporting both our persons served and team members in that process.

Mountain Plains Community Services Society of the North (MPCSSN) is a non-profit, charitable society governed by a volunteer Board of Directors. Our Society has operated numerous diverse community and family-focused programs designed to address the unique needs of the Northwest region of Alberta since 1972. Currently, we offer services in partnership with Alberta Health Services and Alberta Human Services.

  • Salary $53,624.00 - based on a 5 day a week 8 hour day schedule, typically ranging from 10am - 6pm and 12pm-8pm shifts

  • This role is located at our Horizon Home in Sexsmith and supports youth.
Role and Responsibilities:
  • Use creativity and flexibility to create a visual daily schedule based on program and individual needs, focusing time on appropriate group activities and individualized programming
  • Monitor and track individuals’ person-centered goals, and work with program team to ensure deadlines are adhered to and recorded
  • Assist persons-served in creating a network of community supports and involve relevant agencies and family members where appropriate
  • Coordinate and communicate all support strategies and community networking plans with their Program Manager, including monthly reporting on scheduled activities and their therapeutic value to persons served
  • One weekend a month designated on-call for home program
  • Professional collaboration with program stakeholders, including funding bodies (Recovery Alberta), RCMP and other law officials, representatives of the justice system, and community members
  • Champion programming initiatives to Agency members to ensure all staff are supporting persons-served using trauma informed, therapeutic interventions
  • Collaborate with Support Liaison team to implement Agency-wide programming projects and events
  • Attend all meetings, appointments, case-conferences and training as delegated by their Program Manager
  • Participate in monthly supervisions with respective supervisors and attend monthly staff meetings
  • Liaise and collaborate with family members or natural supports of persons-served to ensure a holistic approach to service delivery
  • Support employee onboarding process within the Agency by facilitating Agency tours
  • Complete all other delegated tasks at the discretion of Leadership
Must have
