Job summary

Fraser Health Authority is seeking a clinician for Mental Health & Substance Use Services to support engagement, care coordination, and system navigation across local and regional services in Burnaby.

You will provide client and family‑centered care using trauma‑informed and evidence‑based approaches, conduct screening, intake, assessment, and referrals, and serve as the most responsible clinician for Aftercare when needed.

Qualifications

• Bachelor’s degree in Social Work, Clinical Counseling, Counseling Psychology or a related discipline, and at least two (2) recent years of relevant experience.
• Eligible for registration with the applicable professional association or regulatory body.
• Valid B.C. Driver’s License and access to a personal vehicle for work‑related purposes.

Responsibilities

• Performs initial screening of referrals to determine appropriate treatment based on client needs and urgency.
• Supports the intake process and conducts client assessment via phone or other methods, collecting background information.
• Provides individual, group, and family support using counseling and assessment methods, sharing workload with the interdisciplinary team.
• Delivers first‑level intervention and support until formal treatment is available.
• Manages complex cases, including crisis intervention, and delivers individual and group therapy, developing recovery‑oriented treatment plans.
• Acts as the most responsible clinician for longitudinal Aftercare services when assigned.
• Facilitates client access to community services and programs, liaising with providers and making referrals as needed.
• Provides referrals to diverse community treatment alternatives, including residential programs, family therapy, addiction services, and vocational programs.
• Advocates for clients by providing information on appeal processes and assisting with necessary documentation.
• Offers short‑term counseling and guidance to clients, families, and other caregivers about program services and intake processes.
• Maintains client records using PARIS or other clinical information systems, preparing progress notes and reports.

Detailed Overview

As a clinician on the designated community Mental Health & Substance Use Services or Integrated Response team, you will support engagement, care coordination and system navigation across local and regional services, reducing barriers to care and facilitating timely treatment. Your role includes client and family‑centered care using a trauma‑informed and evidence‑based approach, conducting initial screening, intake, assessment, treatment recommendations, referral, follow‑up, group education, and outreach, as well as serving as the most responsible clinician for Aftercare services when assigned.

Responsibilities

• Provides individual, group, and family support using counseling and assessment methods, sharing workload with the interdisciplinary team.
• Delivers first‑level intervention and support until formal treatment is available.
• Manages complex cases, including crisis intervention, and delivers individual and group therapy, developing recovery‑oriented treatment plans.
• When required, acts as the most responsible clinician for longitudinal Aftercare services, facilitating case management and connecting clients to community resources.
• Facilitates client access to appropriate community services and programs, liaising with providers and making referrals as needed.
• Provides referrals to diverse community treatment alternatives, including residential programs, family therapy, addiction services, and vocational programs.
• Advocates for clients by providing information on appeal processes and assisting with necessary documentation.
• Offers short‑term counseling and guidance to clients, families, and other caregivers about program services and intake processes.
• Maintains client records using PARIS or other clinical information systems, preparing progress notes and reports in compliance with policies and the Freedom of Information Act.
• Reports unsafe situations promptly in accordance with health and safety requirements.
• Develops and maintains links with other facilities and community resources to improve access, integration and coordination of services.
• Collects and reports relevant data and statistics on intakes, referrals, and priorities to coordinate managers.
• Participates in quality improvement, risk management, and program development activities.
• Collaborates in team planning, meetings, committees, and corporate initiatives.
• Engages in ongoing education to maintain current practice and knowledge of relevant legislation.
• Performs other related duties as assigned.

Qualifications

Education, Training and Experience

Bachelor’s degree in Social Work, Clinical Counseling, Counseling Psychology or a related discipline, and at least two (2) recent years of relevant experience with clients and families affected by mental illness and substance use disorders. Eligible for registration with the applicable professional association or regulatory body. Valid B.C. Driver’s License and access to a personal vehicle for work‑related purposes.
