Job summary

Fraser Health is hiring a casual Speech Language Pathologist to join the Rehabilitation team at Queen's Park Care Centre in New Westminster, BC. The role delivers outpatient and inpatient speech language pathology services and works with an interprofessional team to provide patient-centered care.

Responsibilities include assessment, intervention planning, referrals, documentation, and program quality activities.

Qualifications

• Current registration with the College of Health and Care Professionals of BC.
• Two years of recent speech-language pathology experience in inpatient or outpatient hospital rehabilitation settings.
• Strong knowledge of SLP theory and practice within patient- and family-centered care.

Responsibilities

• Assess potential clients' communication, speech and swallow abilities; review records and interview patients.
• Develop evidence-based, outcome-focused intervention plans from assessment findings.
• Coordinate referrals and prioritize outpatient rehabilitation requests with the care team.
• Screen for priority interventions and facilitate referrals to related services.
• Educate clients, families and clinicians on outpatient rehabilitation policies and resources.
• Maintain client records and prepare reports and statistics.
• Share SLP knowledge with the care team and participate in interprofessional meetings.
• Participate in quality improvement, safety initiatives and research projects.
• Support program activities to enhance care delivery and evidence-based practice.
• Perform other duties as assigned.

Detailed Overview

Provides speech language pathology services in accordance with the College of Health and Care Professionals of BC standards of practice, Fraser Health mission, values and speech language pathology acute care standard, and in collaboration and partnership with the interprofessional care team in the provision of patient and family centered care. For clients requesting outpatient rehabilitation services, provides initial client assessment, consultation and intervention based on client needs. Reviews referrals, determines initial services required and utilizing established standards and protocols, prioritizes and confirms referrals as appropriate. Prepares and maintains client records and provides data and statistics. Participates in approved speech language pathology and interprofessional research, special projects, and program quality and safety activities.

Responsibilities

1. Provides speech language pathology assessment of the potential client's communication, speech and swallow functional abilities, needs, and goals by methods such as reviewing records, gathering information, interviewing the patient, identifying patient and speech language pathologist expectations, selecting and applying evidence-based assessment methods, and consulting with members of the health care team.
2. Analyzes the potential client's needs and develops an evidence-based, outcome-focused intervention strategy, utilizing assessment findings.
3. Participates in the interprofessional care planning process for clients by reviewing, prioritizing, and processing requests for outpatient rehabilitation services from potential clients and other health care professionals. Determines initial services required and utilizing established standards and protocols, prioritizes referrals based on client needs and outpatient clinic capacity; accepts and confirms referrals where appropriate.
4. Participates in maintaining seamless quality client care by screening for priority of intervention, redirecting to appropriate rehabilitation services, and identifying and facilitating referrals to appropriate health care/community programs.
5. Educates clients/families/caregivers/healthcare professionals regarding outpatient rehabilitation intake policies, procedures and processes, general rehabilitation resources and the availability and/or eligibility for services.
6. Maintains a variety of records including client files by gathering information, entering data into computerized databases and/or documenting information into approved formats. Compiles statistics, summarizes information into written reports, and prepares correspondence and reports as requested.
7. Acts as a speech language pathology resource by sharing speech language pathology information to other members of the health care team and attends meetings as a representative of speech language pathology.
8. Participates in team/program activities required to enhance and optimize the delivery of care and to facilitate evidence-based practice. Promotes quality and safety principles, identifies processes/procedures that require improvement and provides recommendations for review.
9. Participates in approved speech language pathology and interprofessional research, special projects, and committees including professional practice councils and clinical decision support tool development.
10. Performs other related duties as assigned.

Qualifications

Education and Experience

Current registration with the College of Health and Care Professionals of BC. Two (2) years recent speech language pathology experience working in an inpatient or outpatient hospital rehabilitation setting.

Skills and Abilities

• Knowledge of speech language pathology theory and practice within a patient and family-centered model of care.
• Knowledge of the CSHHPBC standards of practice and practice guidelines for clinical practitioners.
• Knowledge of communication and dysphagia standardized tests, measures, use of equipment and related techniques.
• Broad knowledge of other health care professions and their role in patient care.
• Understanding of outpatient rehabilitation services and how clients can benefit from these services.
• Ability to communicate effectively and respectfully with clients, families, caregivers, members of the interprofessional care team, both verbally (one-on-one and in groups) and in writing.
• Ability to teach and provide effective consultation to clients, families, caregivers and other health care professionals.
• Ability to work independently and in collaboration with others.
• Ability to establish workload priorities and adjust to unexpected events.
• Basic knowledge of research process and methodology.
• Ability to operate related equipment including relevant software applications.
• Physical ability to perform the duties of the position.
