Job summary

Fraser Health in Abbotsford is seeking a Social Worker for the Toxic Drug Response and Priority Populations (TDRPP) within the Integrated Homeless Action Response Team (IHART). You will provide psychosocial assessments, brief therapeutic counseling, and coordinate care with community partners.

Reporting to the Clinical Practice Lead, you’ll work within an interdisciplinary team to develop care plans, support clients and families, and facilitate connections to housing and health services.

Qualifications

• Current full registration with the BC College of Social Workers.
• Bachelor of Social Work from an accredited School of Social Work.
• Two years' recent related experience in a complex health care environment.

Responsibilities

• Conducts comprehensive social work and psychosocial assessments.
• Provides brief therapeutic counseling to clients and families.
• Collaborates with the interdisciplinary team on care plans.
• Maintains patient records and prepares progress notes/reports.
• Coordinates with community services to connect clients with resources.
