Position Overview

Primary Care Social Worker – Registered Social Worker with Northern Health Primary Care Department in Terrace, BC. The role focuses on long‑term care and integration of interprofessional primary care services.

Key Responsibilities

• Comprehensive assessment and care planning for individuals, families, and communities.
• Direct psychosocial care, individual counselling, group work, and system navigation.

Qualifications

• Bachelor’s Degree in Social Work from an accredited university.
