Position Overview
Primary Care Social Worker – Registered Social Worker with Northern Health Primary Care Department in Terrace, BC. The role focuses on long‑term care and integration of interprofessional primary care services.
Key Responsibilities
- Comprehensive assessment and care planning for individuals, families, and communities.
- Direct psychosocial care, individual counselling, group work, and system navigation.
- Advocacy, education, and linkage to relevant community and specialized resources.
- Facilitate client self‑management and support continuity of care across settings.
Qualifications
- Bachelor’s Degree in Social Work from an accredited university.
- Minimum two years of recent related experience in health care or equivalent combination of education, training, and experience.
- Current registration with the British Columbia College of Social Workers.
- Valid BC driver’s licence.
