Assisted Living Alberta is dedicated to Enhancing Care in the Community, helping people be healthy, well and independent in their homes and communities. For more information Assisted Living Alberta. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Social Worker II to join our Supportive Living team to provide care and support to adults and seniors residing in congregate living settings. Supportive Living is a non-institutional alternative for those who are no longer able to live in their own home, which focuses on health promotion, well-being, independence and maintaining functional abilities. As a social Worker II in the Supportive Living Allied Health, you will work with clients and Specialty Services Team of professionals with refined interpersonal skills, well-developed clinical knowledge and experience, and self-direction.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications
  • Completion of bachelor's degree in Social Work.
  • Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications
  • Basic Cardiac Life Support- Healthcare Provider (BCLS HCP).
  • Communication skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment.
  • Responsible to provide proof of active registration with the Alberta College of Social Workers (ACSW) and to maintain registration throughout the duration of employment.
  • The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs.
  • At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment.
  • You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
Preferred Qualifications
  • Education or training in mental health or geriatrics is an asset.
  • Palliative Care experience.
  • Connect Care experience.
  • Recent experience working in the community with older adults who have complex needs.
