Job summary

Vancouver Coastal Health is seeking a Social Work Case Manager to deliver clinical and consultative social work services within a patient- and family-centred framework. You’ll conduct psychosocial assessments, provide counselling, maintain records, and coordinate care with a multidisciplinary team to support clients with complex health needs.

Qualifications include an MSW, 3 years of related case management experience, and active BC social worker registration.

  • Current full registration with BC College of Social Workers.
  • Valid BC Driver’s License. Local travel requires personal vehicle.

Responsibilities

  • Conduct functional and psychosocial assessments for clients and families.
  • Provide individual, family/caregiver, and group counselling.
  • Maintain accurate client records and attend interdisciplinary conferences.
  • Liaise with multidisciplinary team, physicians and community resources.
  • Develop, implement and evaluate care plans and interventions.
  • Coordinate ongoing care and support transitions across the continuum.

Job description

The salary range for this position is CAD $44.85/Hr. - CAD $56.02/Hr.

Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, the Social Work Case Manager provides case management services and clinical and consultative social work services to a select population of clients and families/care givers experiencing brittle support systems, family conflict, social functioning, and mood and behaviour issues. Duties include conducting functional and psychosocial assessments, providing individual, family/care giver and group counselling services, maintaining client records and attending interdisciplinary clinical/case conferences. Identifies applicable community resources for clients, contributes to the effectiveness and development of community resources and plans with clients for their care and support in accordance with their health needs and chronic disease and self care principles. Within a multidisciplinary team context, provides continuity of care across the continuum by liaising with team members, developing/evaluating care plans, coordinating and implementing clinical interventions, evaluating client outcomes based on client goals for care and coordinating ongoing care and supporting transition. Consults, confers, and collaborates with multidisciplinary team members, physicians, educational resources and other health care professionals to achieve excellence in client-centred education and research.

Education & Experience
  • Master’s Degree in Social Work.
  • Three (3) years’ recent, related experience in case management working with geriatric clients and adults with chronic and acute illness or an equivalent combination of education, training and experience.
  • Current full registration with the British Columbia College of Social Workers.
  • Valid BC Driver’s License.
  • Local area travel requires the use of personal vehicle.

  • Demonstrated knowledge and demonstrated ability to apply chronic disease management and health promotion principles.
  • Demonstrated knowledge of case management practice within a client and family centred care model for clients with complex health care needs.
  • Demonstrated knowledge of Indigenous Cultural Safety Education.
  • Demonstrated ability to apply systems theory to practice in Social Work assessment and intervention.
  • Demonstrated ability to conduct social and psychosocial assessments.
  • Demonstrated knowledge of systems thinking as it relates to the social work process.
  • Demonstrated ability to utilize mediation, conflict resolution and counselling skills.
  • Demonstrated knowledge of evidence based practice related to care of clients needing supportive case management.
  • Demonstrated knowledge of other health disciplines and their role in client care.
  • Demonstrated knowledge of other health and community resources related to the care of clients and care givers.
  • Demonstrated knowledge of adult education principles, methods and tools.
  • Broad knowledge of research process and methodology.
  • Demonstrates ability to problem solve using sound judgment in applying critical thinking skills within safe limits of client care.
  • Demonstrated ability to provide leadership and work direction.
  • Demonstrated ability to teach, coach and facilitate clients and families/care givers/care givers individually or in group settings.
  • Demonstrated ability to develop and implement project action plans and accomplish objectives.
  • Demonstrated ability to work independently.
  • Demonstrated ability to work collaboratively as a member of a interdisciplinary team.
  • Demonstrated ability to effectively communicate orally and in writing.
  • Demonstrated ability to communicate with, and deal effectively with co-workers, physicians, other health care staff, clients and their families/care givers, and staff of outside agencies.
  • Demonstrated ability to effectively deal with conflict resolutions.
  • Demonstrated ability to adjust to new or unexpected events.
  • Demonstrated ability to plan, organize and prioritize work in collaboration with others.
  • Demonstrated ability in conducting psychosocial/functional environmental assessments.
  • Demonstrated skill and ability in the use of equipment, supplies and materials utilized in the provision of community based care service.
  • Basic computer literacy to operate a computerized client care information system.
  • Physical ability to carry out the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
