The Social Worker Case Manager is responsible for intervening with patients who have complex psychosocial needs. Working under the direction of the Director of Case Management, the social work case manager performs hospital social work duties, provides timely and appropriate discharge planning service to assure continuity of care, provides consultation and collaboration with patients, families, health care providers, and other systems involved ion patient care. This position will implement coordinated, interdisciplinary plans of care; serve as a resource in identified specialty areas; work closely with the Medical Staff; serve as a patient advocate; develop strategies to improve care delivery and function as an integral part of the care delivery team.

Qualifications
  • Education: Bachelor’s degree in social work or closely related Human Services filed, required. Master’s degree preferred.
  • Experience: 2 years direct experience in healthcare/social worker required
  • Required skills: Literate in use of software and information systems
  • Preferred Skills: Case management certification, LSW, and hospital related social work experience preferred.
