You will support clients in navigating health determinants while providing mental health services in line with contemporary practices. Forensic Assessment & Community Services (FACS) has an exciting opportunity for a diploma level Registered Social Worker. Reporting to the Care Manager of FACS, this position will act as a member of multidisciplinary teams that provide treatment services that are evidence and recovery based, aligned with best practice approaches, policy, legislation, and designated standards of practice within the context of an outpatient, forensic community mental health clinic. The successful candidate will be responsible for supporting clients within the clinic with navigating social determinants of health, accessing resources, completing various applications, and supporting with transition of care when appropriate. The successful candidate will collaborate with colleagues in order to provide input and participation in client treatment plans and progress. This position works closely and collaboratively with community stakeholders, supporting with case management planning and follow‑up. This will include but is not limited to clients with developmental delays, acute mental health issues, and complex social needs. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker I, you will be a member of a multi‑disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families' short‑term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.
Completion of diploma in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
BLS required. NVCI or immediate registration into the next available course is required.
Familiarity with electronic medical records (Connect Care), virtual therapy platforms (Zoom), and comfort with Microsoft Office programs will all be considered assets. NCI. Understanding of intimate partner violence, family systems, inappropriate sexual behaviours, and legal dispositions will all be considered an asset.
