Job description

As part of the Nursing Workforce for the Care Home, the Senior/Staff nurse supervises a team of Enrolled Nurses, Nursing Aides/Assistants and Healthcare Assistants; and oversees the delivery of required and optimal level of healthcare services, based on the defined care model, to residents in the home(s).

You will be posted to Woodlands Care Home.

Primary Responsibilities
  • Assesses residents’ needs on admission and on daily basis throughout their stay at Nursing Home.
  • Plans and implements care according to the assessed needs.
  • Evaluates and monitors residents’ progress according to planned care.
  • Ensure all documentation of residents’ case is maintained appropriately.
  • Co-ordinate with Enrolled Nurses to implement care plan such as medication, hygiene care, social and physical activities for the residents.
  • Participates actively in doctor’s round and carries out treatments and investigations as prescribed.
  • Collaborates and discusses residents’ progress with the doctor and other healthcare providers (Rehab and Medical Social Worker), to ensure holistic delivery of care for residents.
  • Demonstrates clinical skills, knowledge, and critical thinking in the delivery of care to residents. To advocate early medical attention, when there is a decline in functional and cognitive abilities of the resident.
  • Demonstrates proper use and care of equipment, and economical usage of consumables and supplies.
  • Demonstrates competency in all core competencies and unit specific programs implemented by Nursing Administration.
  • Provides resident/family with regular update on residents’ health conditions and facilitate family conference when required.
  • Displays compassion, a positive and caring attitude towards residents/family members, as well as to the peers and supervisors.
  • Ensure compliance to operating procedures and policies to maintain quality clinical standards.
  • Report discrepancies in nursing procedure and policy to NM / SNM and perform incident reporting activities including investigation, report writing and communicating response to the family members involved.
Secondary Responsibilities
  • Oversee and supervise new joiners, junior staff (e.g., NA & HCA) to take stock of their capabilities, to efficiently leverage on their knowledge, skills, and abilities.
  • Conduct counselling session for care staff who may be facing trouble with work or have personal issues that might be affecting their work (ad-hoc basis).
  • Facilitate and assist in planning of regular ward meetings.
  • Identify and assess nursing service problems and assist nurse manager in the resolution of the problems.
  • Keep abreast with the current nursing and healthcare practices (e.g., in-services, case conference, journal etc).
  • Conduct periodical audits (Internal Quality Audit) as well as quality improvement audit to ensure that care staff are performing and meeting clinical standards.
  • Conduct projects and/or research on current issues and in the healthcare sector (e.g. infection control, quality improvement) as assigned by NM/SNM.
  • Senior Staff Nurses are expected to hold expanded roles, as assigned by the Nursing Management Team.
Skills and Knowledge
  • Candidate must possess at least a Degree/Diploma in Nursing from a recognised institution as approved by Singapore Nursing Board.
  • Nurses with post basic/advance diploma in Mental Health or those with previous experience in psychiatric setting are welcome to apply.
  • Valid Practising License with Singapore Nursing Board (SNB).
  • Possess good clinical skills and knowledge.
  • Good interpersonal and communication skills.
  • Customer service-orientated: Compassionate, positive, and caring nature.
  • Good team player who is also able to work well independently.
  • Self-motivated and result oriented.
  • Be able to work rotational 12-hours shift.
