Therapia is recruiting a mobile Occupational Therapist to provide in-home assessments and interventions to adult clients in Brampton, ON. You will work independently within Therapia's care network, with back-end support and scheduling.

The role requires OT registration in Ontario, and a self-motivated, travel-capable professional who can manage a flexible caseload in clients' homes.

Qualifications

  • Registered and in good standing with the College of Occupational Therapists of Ontario (COTO).
  • Minimum 1 year of clinical experience (hospital, clinic, community, or home care).
  • Experience conducting functional assessments and planning interventions.
  • Access to a reliable vehicle and valid driver's license.
  • Willingness to travel within your region.

Responsibilities

  • Conduct comprehensive home OT assessments evaluating function, mobility, safety, ADLs and IADLs.
  • Develop individualized treatment plans based on diagnosis, environment and goals.
  • Deliver functional interventions for self-care, mobility, transfers, energy conservation and cognition.
  • Assess home safety and recommend modifications and equipment.
  • Educate clients and families on safe mobility, ADLs and energy management.
  • Monitor progress and adjust plans as needs change.
  • Maintain clinical notes and documentation per regulatory standards.
  • Collaborate with physicians, therapists and caregivers to ensure continuity of care.
  • Schedule appointments independently via Therapia's platform.

Your Role

As a Therapia Occupational Therapist, you'll provide home occupational therapy assessments and interventions to adult clients recovering from surgery, injury, illness, or managing chronic conditions. You will work independently while being supported by our collaborative network and backend systems.

Key Responsibilities
  • Conducting Comprehensive Assessments:
    Perform in-depth occupational therapy assessments to evaluate a client's functional abilities, mobility, cognition, safety, and ability to participate in activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Identify barriers to independence and meaningful participation.
  • Developing Individualized Treatment Plans:
    Create tailored occupational therapy care plans based on each client's diagnosis, functional needs, environment, and personal goals. Develop practical interventions that support independence, safety, and participation in daily activities.
  • Delivering Functional Interventions:
    Provide evidence-based occupational therapy interventions addressing areas such as self-care, mobility, transfers, functional activities, energy conservation, cognitive strategies, and return to meaningful daily routines.
  • Assessing Home Safety & Accessibility:
    Evaluate clients' home environments to identify safety risks, accessibility barriers, and potential challenges to independence. Make appropriate recommendations for home modifications, adaptive strategies, and equipment.
  • Recommending Adaptive Equipment:
    Assess clients' needs for assistive devices and adaptive equipment and provide recommendations to improve safety, function, and independence in the home and community.
  • Educating Clients & Families:
    Provide education and practical strategies to clients, families, and caregivers regarding safe mobility, ADLs, energy conservation, cognitive strategies, equipment use, and strategies to maximize independence.
  • Monitoring Progress:
    Track client outcomes, evaluate progress toward functional goals, and modify treatment plans and interventions as needed to ensure care remains responsive to the client's changing needs.
  • Maintaining Accurate Documentation:
    Complete clinical notes, assessment forms, progress reports, and other required documentation in accordance with regulatory standards and Therapia's documentation protocols.
  • Communicating with the Care Team:
    Collaborate with clients' physicians, physiotherapists, nurses, social workers, caregivers, and other healthcare professionals as needed to support continuity and quality of care.
  • Ensuring Client Safety:
    Identify and address risks related to falls, transfers, functional mobility, cognitive impairment, and the home environment. Follow appropriate infection control and safety protocols at all times.
  • Managing Your Schedule Independently:
    Coordinate your appointments directly with clients via Therapia's platform, allowing you to work efficiently and autonomously within your preferred service area.
Qualifications
  • Registered and in good standing with the College of Occupational Therapists of Ontario (COTO)
  • Minimum 1 year of clinical experience (hospital, clinic, community, or home care setting)
  • Experience conducting functional assessments and developing individualized intervention plans
  • Strong communication, clinical reasoning, organizational, and time management skills
  • Comfortable working independently and traveling within your region
  • Access to a reliable vehicle and valid driver's license
  • Professional liability insurance
  • Experience in community-based, home care, orthopedic, neurological, post-surgical, or geriatric occupational therapy is a strong asset
