Fraser Health Authority in British Columbia invites applications for the Client Care Coordinator role within the UPCC/PCN. You will provide clinical supervision, support to nursing staff, and oversee day-to-day operations to ensure timely urgent care and smooth patient flow.

Reporting to the Manager, you will mentor staff, coordinate education, participate in quality improvement and risk management, and collaborate with interdisciplinary teams to deliver person-centred, trauma-informed care

Qualifications

• Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• Three years of related clinical experience including supervisory experience or equivalent.
• Ability to supervise, coordinate staff, and support education and quality improvement.

Responsabilités

• Leads daily intake and patient flow with the care team.
• Supervises nursing and support staff and coordinates day-to-day operations.
• Mentors staff and promotes inter-professional collaboration to ensure quality care.
• Supports education priorities and develops learning programs for staff.
• Participates in quality improvement and risk management activities.
