Alberta Health Services in Lloydminster is hiring an Advanced Nurse Practitioner to support two Continuing Care Homes: Lloydminster Continuing Care Centre (60 beds) and Dr. Cooke Extended Care Centre (50 beds).

The NP will provide full-scope primary care to older adults, working autonomously and with a Primary Care Provider. Role includes assessing, diagnosing, ordering tests, prescribing treatments, and performing procedures, within the MRHP model, while coordinating with an interdisciplinary

Qualifications

• Master's degree in Nurse Practitioner program.
• Active CRNA registration on the Nurse Practitioner Registrar, including Provisional Graduate licensure.
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
• Preferred: Certified Capacity Assessor with experience in Continuing Care and Connect Care.

Responsabilités

• Provide full-scope primary care to older adults in two facilities.
• Assess, diagnose, order and interpret diagnostics, and prescribe treatments.
• Perform procedures within the MRHP model and coordinate with Primary Care Providers.
• Collaborate with physicians, nursing, allied health and community partners to ensure continuity.
