Job summary

A community health services provider in Toronto is seeking an RPN Team Lead to oversee and coordinate care for clients as part of a transdisciplinary team. The successful candidate will have a Registered Practical Nursing diploma and at least 5 years of experience in community-based healthcare. Qualifications

• Minimum 5 years experience in community based health care system.
• Registered with College of Nurses in province of practice.
• Team Lead or supervisory experience preferred.

Responsibilities

• Supervises the work of the team according to planning guidelines.
• Provides complete, relevant assessments of the client/family.
• Acts as a clinical resource to the service delivery team.
