Job summary

Ontario Shores Centre for MHS in Whitby, ON seeks a Registered Practical Nurse for the Durham ACT Team. You will perform mental health assessments, participate in a multidisciplinary team, and provide assertive community treatment and rehabilitation services.

You will develop treatment plans with clients, families, and the team; manage medications and monitor adherence; deliver crisis intervention and support recovery in both hospital and community settings.

Qualifications

• Registration with the College of Nurses of Ontario in good standing.
• Current CPR – Health Care Provider (HCP) certification.
• Knowledge of CNO Standards of Practice and reflective practice.
• Knowledge of Canadian Standards for Psychiatric–Mental Health Nursing and related laws.
• Excellent interpersonal and oral/written communication skills.
• Experience with recovery principles and psychosocial rehabilitation.
• Ability to work independently and in a multidisciplinary team.
• Computer literacy; Meditech experience preferred.
• Experience in mental health and acute psychiatry settings.

Responsibilities

• Perform mental health and physical assessments on clients.
• Participate as a mental health professional on a multidisciplinary team.
• Provide assertive community treatment and client-centered rehabilitation.
• Develop goals and plans with clients, families and the team.
• Deliver medication administration and monitor compliance.
• Provide crisis intervention and support to clients and families.
• Support daily living activities and harm reduction in practice.
• Monitor for deterioration and prevent unnecessary hospitalization.
