Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents in a long-term care setting. The role involves directing Personal Support Workers and collaborating with physicians and the health care team to ensure high-quality resident care.

It requires a strong background in geriatric or rehabilitation nursing, knowledge of relevant Canadian acts, and current CNO certification.

Qualifications

  • Sound knowledge of geriatric, rehabilitation or long-term care nursing, including activation, nursing, and personnel administration.
  • Familiarity with the Fixing Long-Term Care Act and related health acts and regulations.
  • Current Certificate of Competence from the College of Nurses of Ontario and in good standing.

Responsibilities

  • Collaborate with RPN to implement AgeCare policies on the unit.
  • Provide input to revise procedures within the residence and unit.
  • Communicate with physicians and the health care team, including external partners.
  • Notify physician of significant resident status changes when RN is unavailable.
  • Work with health care team to meet residents’ care needs.
  • Direct Personal Support Workers on the care team.
  • Participate in shift handoffs ensuring status changes are communicated.
