Qualifications

  • 3 - 5 years of job-related experience or equivalent education/experience.
  • Registration with the College of Nurses of Ontario (CNO) is required.
  • G or G2 Driver's license, access to a reliable vehicle.
  • Clear Background and Vulnerable Sector Check.

Responsibilities

  • Provide coordination, supervision, and coaching to Community Support Workers.
  • Develop and maintain effective Care Plans for Community Health Services.
  • Keep abreast of policies and employee performance monitoring.
  • Promote and market Community Health Services.

Excellent English verbal and written communication skills
Thorough knowledge of case management skills
French language skills

Education

Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
Current CPR Certification
