Qualifications

• 3 - 5 years of job-related experience or equivalent education and experience.
• Registration with the College of Nurses of Ontario (CNO) required.
• Experience in a community healthcare environment is an asset.

Responsibilities

• Provide coordination, supervision, and coaching to Community Support Workers.
• Develop and maintain effective Care Plans for Community Health Services.
• Promote and market Community Health Services.

Proven computer skills

Knowledge of case management skills and nursing processes

French language skills

Education

Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma

Current CPR Certification
