Responsibilities

• Identify and address ongoing resident needs to promote the highest quality of care.
• Conduct resident evaluations and document wellness visits.
• Monitor and follow up on resident incidents.
• Assist with medication management including transcribing orders.
• Contribute to engagement and improvement initiatives.
