Rapid Access Addiction Medicine (RAAM) & Primary Care RN

1 FTE (35 hours per week)

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a compassionate, committed individual to work with the Primary Care team as well as in the Rapid Access Addiction Medicine (RAAM) Clinic.

This position will provide nursing services within NorWest CHC’s Interprofessional Primary Health Care team and the RAAM team. Within the RAAM clinic, the RN provides low-barrier addictions care to clients and their families. The approach to care is strengths-based and rooted in harm reduction and evidence-based practice. Within the Primary Health Care team, this RN will assist in the delivery of primary care services to vulnerable populations through education, individual support, screening, and outreach.

Eligible for the Community Commitment Program for Nurses (CCPN) that provides $35,000 in grant funding to eligible nurses in exchange for a 2 year commitment.Click here for eligibility criteria.

QUALIFICATIONS:

  • RN with current College of Nurses of Ontario registration.
  • Minimum 3 years nursing experience, ideally in a combination of hospital, community and/or public health settings
  • Addictions nursing experience required.
  • Ability to work flexible hours including evenings and weekends as required.
  • Strong organizational, communication and leadership skills.
  • Ability to work both independently, and collaboratively, within an interprofessional team environment.
  • Perform clinical activities according to the College of Nurses standards for nursing practice.
  • Utilizes professional assessment skills to identify physical, mental, and psychosocial needs of clients and families.
  • Knowledge and proficiency in current evidence based practices of primary care delivery, with an emphasis on health promotion and harm reduction.
  • Experience and flexibility to respond to fluctuating workload and changing environment
  • Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
