Job summary

Progressive Housing Society in Burnaby is seeking a Community Registered Nurse to join our mental health team on a part-time basis. You will provide clinical care in the community, aligned with Recovery Plans and in collaboration with Fraser Health Authority's BMHSU program.

Key Overall Objective of the position

The Community Registered Nurse (RN) functions as a member of the Progressive Housing Society’s mental health team and applies best practices to provide competent, safe, and ethical care for individuals living with mental health concerns in the community, including those in an Assisted Living residential setting. The Community RN provides care that includes clinical supports according to an individual’s Recovery Plan in collaboration with Fraser Health Authority’s (FHA) Burnaby Mental Health and Substance Use (BMHSU) program. The Community RN will work with a Harm Reduction philosophy and support best practices and guiding principles as directed by FHA.

Under the direction of the Manager, and in collaboration with clinical and community partners, the Community RN provides guidance and supervision to teams delivering mental health supports, as well as performing assessments, and the planning and implementation of personal and nursing care to clients. The RN also oversees the medication administration program for the Assisted Living site. The RN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the College of Registered Nurses of British Columbia (CRNBC) and according to organizational policies, procedures, standards and unit-specific protocols.

The Community RN takes a collaborative and team-based approach in program delivery and problem solving to achieve excellent outcomes for clients and programs. This approach involves nurturing a team environment of continuous learning, effective communication, and ensuring fulfillment of service standards within operating budgets.

The Community RN supports individuals to achieve and maintain optimal health by performing the duties and responsibilities below.

Key Accountabilities

Provides direct clinical care and services to individuals, groups and the community. This may include nursing care and services in the areas of population and community health, basic mental health, chronic disease prevention and management, geriatrics, and other areas of nursing.

• Assesses and monitors clients actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Supports activities of daily living and where appropriate encourages client participation and increasing independence in these areas.
• Establishes and maintains a therapeutic relationship with the client and their families through the use of interpersonal and interviewing techniques ensuring client choice and autonomy in decision-making and care planning.
• Contributes to the development and ongoing changes to the individualized Recovery Plan, including attending and providing input into multidisciplinary care planning meetings. Advocates for the interest of the client.
• Acts as a role model by demonstrating empathy, trust, and respect in all interpersonal relationships while also demonstrating strong professional values as needed in collaborative decision making.
• Promotes active care coordination and efficient resource utilization by monitoring care planning, goal setting, evaluation and documentation.
• Promotes and coordinates teamwork, facilitates team decision making and problem solving processes. Plans, prioritizes and problem solves issues related to workload distribution. Provides clinical process guidance, including the support of evidence-based decision making, to assigned primary care and interdisciplinary team staff.
• Coordinates and leads the day-to-day operation of shared living sites as directed by the Manager, within the language of the collective agreements and Progressive Housing Society Policies and Procedures. As directed, prepares monthly work schedules, approves leaves and overtime, arranges for casual staff as required, and verifies payroll information.
• Oversees effective medication administration program. Administers oral, subcutaneous, and intramuscular medications including PRN medications in accordance with organizational and pharmaceutical policies and procedures. Trains and monitors staff in medication administration.
• Participates in the selection and hiring of new staff, taking into consideration the needs of the collaborative team environment and the input of the team.
• Acts as a preceptor or mentor to guide the clinical experience of assigned new nursing staff and students as applicable; provides input to the designated family physician(s), Nurse Practitioners and/or program Manger as requested on team member’s progress in achieving established standards of care.
• Contributes to the development of reference and other resource materials required to guide practice in accordance to the established quality standards.
• Identifies learning and development needs of staff and works to implement plans.
• Collaborates with physicians and/or Nurse Practitioners, the interdisciplinary team and other services to develop, implement and evaluate comprehensive care plans and interventions for people and their families using systematic assessment and best practices/evidence based standards involving the person and, where appropriate, the family in the context of their community.
• Performs other related duties as assigned.

Qualifications

• Must have Graduated from a recognized program for Registered Nurses
• Must have three (3) years of recent, related experience; or an equivalent combination of education, training and experience.
• Current, full practicing licensure with CRNBC
• Must have 1+ years recent related work experience with vulnerable populations
• Previous experience supporting individuals with mental health concerns is required.
• Life experience may be considered in part for related experience
• A valid BC driver’s license, Abstract, and vehicle for work use
• First Aid, Provincial Violence Prevention, motivational interviewing and trauma-informed care an asset
