Qualifications

• 2+ years of clinical experience.
• Existing relationships in hospitals or community health networks.
• Valid driver’s license and willingness to travel across the Lower Mainland.

Responsibilities

• Build and nurture relationships with discharge planners and physicians.
• Educate referral sources about services through presentations and meetings.
• Conduct clinical assessments for prospective clients.
• Guide families through care options and coordinate service start dates.
• Maintain records in CRM systems.
• Provide Nursing services on an as-needed basis.
• Supervise and train other nurses and team members.
