Flexible Part‑Time Role Paid per Intake Bonus Incentives

We are looking for a compassionate and professional Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team and support our growing home care agency. This role focuses on conducting intake assessments for new clients, helping us understand their care needs and setting them up for success with our services. It will also include follow‑ups with clients as requested/necessary.

What You’ll Do
  • Conduct in‑home or virtual client intakes and assessments
  • Gather medical history, medications and daily living needs
  • Document care recommendations and develop initial care plans
  • Collaborate with office staff and caregivers to ensure a smooth start to care
  • Build rapport with clients and families, making them feel supported from the very first meeting
  • Follow up with clients on their care journey, updating care plan as necessary and ensuring exceptional quality of care.

What We’re Looking For
