Registered Practical Nurse – HART Hub

New Position - REPOST

3-year Full-Time Contract (possibility of extension pending funding)

Salary Range: $58,662 - $64,101

We work with our 11 First Nation community partners and the IFC in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services, and health promotion services.

The opportunity

Under the direction of the HART Hub Regional Manager, the Registered Practical Nurse is responsible to provide clinical support to community members who access the Hart Hub. This role provides direct clinical care within their full scope of practice and participates in the development, implementation, monitoring and evaluation of services for individuals. The Registered Practical Nurse will work within a team of other multi-disciplinary team members to provide services to First Nation and Aboriginal people.

What you need to bring with you
  • Registered Practical Nurse College Diploma and current registration in good standing with the College of Nurses of Ontario is required.
  • Current CPR and First Aid.
  • Experience in mental health, addictions, homelessness services, community health, or related settings is preferred.
  • Training in harm reduction, trauma-informed practice, crisis de-escalation, and Indigenous cultural safety is an asset.
  • Ability to work in non-traditional settings (bed-based centres, drop-in centres, outreach, mobile clinics).
  • Strong interpersonal skills and comfort working with highly marginalized populations.
  • Having knowledge of prescription drug abuse or polysubstance abuse would be an asset.
  • Demonstrated knowledge and experience in using drug assessments and screening tools.
  • Ability for conceptualizing and addressing individual addiction issues from a wholistic perspective.
  • Demonstrated ability to develop and facilitate health teachings and presentations for health promotion and awareness related to prescription and illicit drug abuse.
  • Knowledge in the use of computers, various word processing, spreadsheets and electronic medical record.
  • Must have access to a reliable vehicle and a valid G Class driver’s license.

What to expect?

Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage or cedar may occur.
