Job summary

Women’s College Hospital is seeking a Temporary Casual, Registered Practical Nurse for the Crossroads Clinic in Toronto. The role supports refugee women and families with primary care needs in a fast-paced team setting.

The nurse will provide basic examinations, triage, immunizations, and care coordination. A valid RPN license, BLS, and 3–5 years in primary care are preferred, with EMR experience (TELUS/PSS and EPIC) advantageous.

Qualifications

• Registered Practical Nurse Diploma.
• Current Certificate of Competence (RPN) from the College of Nurses of Ontario.
• Basic Cardiac Life Support certificate.
• A minimum of 3–5 years’ experience in a Primary Care/Family Practice setting.
• Experience with immunizations, prevention and chronic disease management.
• Two years’ experience with Electronic Medical Records (ERM) preferred (TELUS/PSS and EPIC).
• Autonomous nursing decisions and strong patient education are encouraged.
• Multilingual abilities are an asset.

Responsibilities

• Provide direct nursing care, collaborating with health professionals and organizing patient care activities.
• Practice within Standards set by the College of Nurses of Ontario.
• Coordinate with physicians, NPs, social workers and other team members for continuity of care.
• Triage patient calls, provide advice and respond to concerns per standards.
• Conduct basic physical exams and triage for walk-ins and same-day patients.
• Perform procedures such as venipuncture, immunization, TB testing, urinalysis, and throat swabs as needed.
• Assist patients to access community health resources via advocacy and follow-up.
• Record patient information accurately in the EMR.
• Ensure standard of care and follow-up including vaccinations and preventive screening.
• Organize follow-up arrangements/referrals to community services.

Women’s College Hospital Crossroads Clinic is looking for a Temporary Casual, Registered Practical Nurse (up to 6 months)! The Crossroads Refugee Clinic provides primary care to newly arrived refugee women and their families

Type: Replacement

Summary of Duties, but not limited to

• Responsible for providing direct nursing care, collaborating with other health professionals and participating in the organization of activities in the provision of patient care
• Practices within his/her full scope of practice as outlined in the Standards of Practice by the College of Nurses of Ontario.
• Liaises with Physicians, Nurse Practitioners, Socials Workers, and other team members to ensure team and unit functions efficiently and provides continuity of patient care
• Triages patient phone calls, provides advice and responds to patient concerns in accordance with the standards established by the College of Nurses of Ontario

• Conducts basic physical examinations (e.g. vital signs, height and weight, etc.), basic history and clinical triaging for walk-ins and same-day patients in collaboration with other members of the clinical team

• Performs medical and diagnostic procedures such as venipuncture, immunization, allergy injections, TB skin tests, urine dips and BHCG, ear irrigation and throat swabs on as needed basis
• Assists patients in accessing appropriate health care resources in the community through advocacy and follow up care
• Records patient information in a precise, concise and organized manner using an electronic medical record
• Ensures patients receive appropriate standard of care and follow-up (e.g. chronic disease management, well child visits, vaccines, preventative screening, follow-up on abnormal labs etc.), which may involve chart review, recalling patients for appointments etc.
• Organizes necessary follow-up arrangements/referrals to community-based services for patients
• Incorporate a wide knowledge base to facilitate professional responsibilities in the provision of patient care including patient education on topics such as heart health, diet and exercise

Qualifications/Skills

• Registered Practical Nurse Diploma
• Current Certificate of Competence (RPN) from the College of Nurses of Ontario.
• Basic Cardiac Life Support certificate
• A minimum of 3-5 years’ experience in a Primary Care/Family Practice setting in the following clinical practice areas:

• Lifestyle and prevention (i.e. Smoking cessation, alcohol use, diet and exercise)
• Immunizations (baby, child, adolescent and adult)
• Surveillance and screening (i.e. Cancer screening – colorectal, cervical, breast)
• Life cycle clinical management (i.e. Clinical visits – well baby, child, adolescent, maternity, adult and frail elderly)
• Sexual health
• Family violence
• Chronic disease management (i.e. Mental health, diabetes, hypertension, CHD, stroke, TIAs, asthma, COPD, hypothyroidism, epilepsy, cancer)

• 2 years’ experience with Electronic Medical Records (ERM), preferably TELUS/PSS and EPIC
• Excellent computer skills and a minimum of 2 years’ experience with an Electronic Medical Records (EMR) Primary Care system or equivalent experience required.

• Successful completion of a Nursing Physical Assessment Course, Solution Focused Course, and Pharmacology Course, an asset

• Ability to speak one or more of the following languages Spanish, Arabic, Farsi or Dari, Hungarian, Amharic, Tigrinya, and Korea, an asset
• Experience working with local or overseas high needs communities would be an asset.
• Demonstrated ability to make autonomous nursing decisions regarding patient care.
