Job summary

SE Health in Canada is seeking dedicated nurses to provide compassionate home and community care across Peel Region. You will complete assessments, develop care plans and collaborate with a multidisciplinary team to deliver high-quality care.

This role offers flexible schedules, paid training, opportunities for growth, and competitive pay. You will work days/evenings/nights with weekend rotation and maintain your professional standards.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• CPR certification is required.
• Immunization records and background checks are required on hire.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best practices.
• Be the point of care lead when designated for a client caseload in collaboration with the care team.
• Support clients through transitions by connecting with community supports.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
