Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents, in collaboration with the Registered Nurse, and to mentor Personal Support Workers.

The role involves liaising with physicians, following AgeCare policies, and ensuring resident needs are met across shifts while upholding standards of care; applicants must hold a Certificate of Competence in Ontario and be in good standing.

Qualifications

• Experience in geriatric or long-term care nursing.
• Familiarity with applicable Canadian statutes and acts.
• Current nursing license in Ontario and in good standing.

Responsibilities

• In collaboration with the RN, implements AgeCare policies on the unit.
• Liaises with physicians and other health care team members.
• Notifies physician of significant changes in resident status in RN’s absence.
• Provides direction to Personal Support Workers on the care team.
• Participates in shift reports to inform team of changes in resident status.

Provides input into the revision of procedures within the residence and the unit.Liaises with physicians and other members of the health care team including external health care partners.
